The RN Coordinator in the Home Telehealth Program is a professional who coordinates care for a panel of patients throughout the continuum of care to assure that care is timely, appropriate, of high quality and cost effective. A RN Coordinator works closely with the primary care provider (or providers) and other healthcare professionals and team members, other clinics, internal or external services and community agencies. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
